
What is it?
The Consent Form for Bioidentical Hormone Replacement Therapy (BHRT) is a legally required document to obtain patient consent for compounded bioidentical hormones.
It must be signed by the patient before BHRT is prescribed. (Note: You'll use this Form in addition to your regular Informed Consent/Consent to Treat Form.)
It covers areas like:
- description of BHRT.
- potential risks + benefits of BHRT.
- voluntarily consent to receive BHRT.
Who needs it?
All licensed medical practitioners working with patients who have prescription privileges for BHRT including:
- Physicians (all types), including Integrative, Functional and Lifestyle Physicians
- Nurse practitioners
- Physician assistants
- All licensed medical practitioners working in patient relationships who are allowed to prescribe BHRT under their scopes of practice.
