What is it?
The Informed Consent Form (also called Consent to Treat Form) is a legally required document grounded in consent and clarity for your patients to sign.
It must be signed by the patient before/at your first patient appointment.
It covers areas like:

- your expertise and scope of practice.
- voluntarily consent to receive medical treatment.
- payment, communication, and confidentiality policies.
