what is a telehealth consent form?
A Telehealth Consent Form is for you if you’re a licensed practitioner offering medical services and you want to work with patients via Telehealth – like an integrative physician, counselor, social worker or therapist, registered dietician, doctor of chiropractic medicine or licensed naturopathic medicine, or physical therapist, for example. Have your patients sign a Telehealth Consent Form BEFORE you start working with them using Telehealth services to get their full and voluntary consent.
IMPORTANT NOTES YOU NEED TO KNOW ABOUT TELEHEALTH: (More details are shared in the audio guide – be sure to listen!)
1. Use Telehealth ONLY in states where you’re licensed. Know that only a few states allow Telehealth to be used across state lines.
2. In some states ALL licensed medical practitioners may use Telehealth or Telemedicine – but in others only SOME types may do so. Know your state law’s exact definition of “Telehealth” and “Telemedicine.” Each state defines them differently.
3. Some states require a written Telehealth Consent Form, and in others, it’s optional. But in ALL states, using a written consent form to receive Telehealth services is a best practice.
With these points in mind, know that this Telehealth Consent Form will make your Telehealth policies clear and get your patient’s consent so there’s no confusion.

From a PRACTICAL standpoint, every patient needs to understand and agree to your Telehealth policies, know the risks and benefits, and be informed of the Telehealth services BEFORE they receive them.
